Clinical Services OverviewVGHThe VGH Division of Geriatrics provides comprehensive geriatric assessments for hospital inpatients at Vancouver General Hospital. The geriatric consult service supports the medical and surgical teams in the care of complex and frail seniors often with multimorbidity and polypharmacy. Our outpatient Geriatric Rapid Access clinic sees patients from the surrounding community for comprehensive geriatric assessments. Referrals typically come from the Emergency Department, but also other specialty clinics and family physicians in the community. We see patients for pre-operative geriatric assessment in an expedited manner and have a physiotherapy team to help our seniors with gait and balance training. The Falls Clinic sees patients for prevention of falls. Our outpatient clinic also has a Teaching Clinic, providing clinical training for residents, students and fellows from UBC and other institutions. PHC The Division of Geriatric Medicine at Providence Health provides clinical services to Older Adults at 2 hospital sites - St. Paul's and Mount St. Joseph's hospitals. At both sites Geriatricians provide both inpatient and outpatient care (including Home Visits) to Older Adults with a range of health conditions. Outpatient clinics include comprehensive Geriatric Assessment clinics, as well as clinics focused on Cognitive changes/Memory Loss, Falls and Mobility, Continence, Pre-surgical assessment and Geriatric Cardiology. Appointments are made by referral from a Family physician, ER physician or other specialists. Fraser Health Fraser Health geriatricians provide community interdisciplinary care in Specialized Seniors Clinics across the region. At 9 acute care sites, geriatricians also provide inpatient consultation. Geriatricians also participate in quality improvement work at various sites within Fraser Health. Richmond Hospital The Richmond Geriatric Assessment Program (GAP) is a comprehensive geriatric consultation and treatment program for Richmond residents aged 70 and older. The program is staffed by geriatric medicine specialists, nursing, occupational therapists, physiotherapists, and program assistants who coordinate the activities. Referrals must be initiated or approved by a physician. The geriatricians also provide geriatric medicine consultations at the Richmond Hospital. North Shore From Sea to Sky: There is a Geriatric Clinic in West Vancouver and in Squamish. Outreach services exist for Pemberton and Mount Currie if the patient cannot come to Squamish. Reviews of residential care clients are provided at some sites on the North Shore and in Squamish. Consultation service is provided at Lions Gate and at Squamish Hospital. The Geriatricians work informally in teams with Home and Community Care in North/West Vancouver and Squamish and the Sea to Sky corridor, as well as outreach services provided to the Sunshine Coast. VIHA At Island Health, geriatricians and care of the elderly physicians provide geriatric services across the Island. In Victoria, in-patient consultation services are provided to 3 hospitals as well as out-patient clinics at 2 sites. In Duncan and Nanaimo, our division provides both in-patient consultation services and out-patient ambulatory clinics. In Port Alberni, in-patient consultation services are available. In Campbell River, a small number of out-patient consultations and occasional in-patient consultations are accessible but are currently being expanded. Kelowna Kelowna offers a suite of geriatric services, including inpatient consultations at both Kelowna General Hospital and the Geriatric rehabilitation short stay unit. We also have a specialized clinic at the Seniors Health and Wellness Centre and home and care facility visits are available for select requested patients. MAID (Medical Assistance in Dying) assessments and provisions for those with complex geriatric conditions are also provided. |
